We recently distributed a survey asking about your current experiences with Children's Developmental Disabilities Medicaid Benefits (formerly known as Children's Medicaid Redesign.)
We are continuing to examine the issues you all are sharing with us and providing one to one assistance to those of you who have asked to be contacted as well as seeking solutions to common issues.
At this time, however, we're looking to go further in depth with regards to any child who may be waiting for services. Even if you filled out the first survey, we would greatly appreciate you taking the time to fill out this VERY SHORT questionnaire:
Idaho Parents Unlimited's Family to Family Health Information Center on behalf of the Consortium of Idahoans with Disabilities shares the results of these surveys (no personally identifiable information is shared) with the Department of Health and Welfare and the Idaho Legislature in our efforts to ensure positive health outcomes and to improve systems of care for children who have disabilities and special health care needs.
The information you provide is always anonymous and unidentifiable unless you choose to provide us with your contact information. We will follow up with every request.
